Initial Questionnaire

Your provider has the option of creating an initial questionnaire to be completed as part of the registration process.

1. Enter your answers to the best of your ability before proceeding. Questions can be composed of several types:
• Single or Multiple Line Free Text: Simply click the field and type your response to the question as appropriate.
• Drop-down Menu: Select your answer which is provided in a drop-down list of options.
• Checkbox List: Click the checkbox next to the appropriate responses for the question provided.
2. Click Save to complete your registration. The following pop-up message will appear.



Click OK. Once your request has been processed, you will be sent an email confirmation with your user account information.
