Login Information

Before anyone can use the portal, they must register as a user with the appropriate care provider. The registration process is completed in one of two ways:

Providers can register and provide their patients a user log-in account during an office visit.
Patients can register themselves by following a link provided on the Portal Home page. This a 3-step process in which the patient

Configures the log-in information (described below)
Enters their demographic information
Completes any Initial Questionnaire that the provider has configured

Once a patient's registration is complete, they can begin to use the Portal.

New Patient Registration

Navigate to URL provided by your provider. As a prospective portal user, your care provider will have directed you to the Portal Home page either through a link they have provided on their website or in literature they have given you.
Click the Not already a patient with us? Click here to register! link. The Login Information screen (aka, New Patient Registration) will appear.

Use the following descriptions and definitions to complete your registration.
Once you have completed this page, click the Continue link to proceed to the Demographic Information for the new user. (The link appears on the right side of the screen.)

Login Credentials Panel

User Name: Enter any User Name you desire up to 30 characters in length.
Password: Enter your desired Password up to 30 characters. Mix letter case and alphanumeric characters for a more secure password. So, for example, maryjoe1234 is not the same as MaryJoe1234.
Re-enter the Password as a verification.

Login Questions

Each user must create three separate security questions that are used to verify that you are who you say when you log in to the system. You will then be given the opportunity to select an option for saving the answer on each computer you use.

First Question Text: Enter your first question. Use anything that you will remember.
First Question Answer: Enter the appropriate response to the first question.
Repeat steps 1–2 for the second and third questions/answers.
